1. Submit this form

2. Orthogroup customer service will review the product, warranty status,and provide a quote via email or phone for repair of the instrument(s).

3. If a fee is involved, provide approval for the repair via email or verbal. The standard repair fee for non-warranty instruments in $690 and includes:

  • Repair
  • Testing
  • Cosmetic Cleanup
  • Automatic Warranty Renewal (6 months)

4. Return the instrument(s) to:
